Patients wishing to register who live outside this area, are asked to make an appointment with a Partner to discuss their registration.
To register with the surgery:
- Print out and complete the registration form from the link below.

Once you have completed the registration form, please hand it in to our Reception Desk to start the registration process.
- visit reception and collect a paper registration form
Please bring at least one form of Identification (detaild below) with you so that we can positively identify you:
- Birth Certificate
- Studnt ID Card
- Driving Licence
- Paspport or HC2 Certificate
- Home Office App. Reg. Card
Where appropriate, an appointment will be organised for a New Patient Medical, with our Practice Nurse within two months of registering. This health check gives the Practice the opportunity to meet you and to tell you about the Practice, and allows the Practice to gather any relevant aspects of your medical history before your records become available to us.
Third-party Access
As a parent, family member or carer, you may be able to access services for someone else. We call this having Third-party Access. To request third-party access, please either print out the Third-party Access form from the link below and hand it into our Reception desk or alternatively collect a paper copy from our Reception Desk between 8.30am and 5.30pm.

Accountable GP
In Scotland, on the 1st of April 2015 the NHS introduced the concept of having named accountable GPs to ensure that pateints have a named GP within their Practice who is responsible for the coordinating and delivery of all their patient'sappropriate medical services:
- Responsibilities: The named GP is responsible for ensuring that all appropriate services are coordinated and delivered to their patients.
- Patient rights: Patients can still see any GP at the Practice and book appointments with their preferred GP.
- Patient preferences: Practices will try to accommodate a patient requests for a specific GP.
- Patient notification: Patients are informed of their named GP when they first interact with the Practice.
- Patient access: Patients can ask at th Reception Desk for the name of their named GP.
